Because of an abundance of LA in the diet, one tends to assume that the formation of long-chain n-6 PUFAs is adequate. This is far from the truth because the process of converting LA to long-chain metabolites is rate-limited by the activity of ?6-desaturase, whose activity is further modulated by many nutritional and physiological factors. Under these circumstances, even if the intake of LA is adequate, the depressed ?6-desaturase activity may substantially reduce the formation of long-chain n-6 PUFAs, and subse quently, result in a state of functional n-6 essential fatty acid deficiency. Clinical evidence has associated this deficiency with many pathological conditions. Fortunately, this inadequacy may be alleviated by dietary supplementation of ?- linolenic acid (GLA), the ?6-desaturation product of LA. GLA was first isolated from the evening primrose. This herbal plant was tradi- tionally used by the Native Americans to treat various diseases, and later discovered by the European settlers. In the 17th century, this plant was transplanted to Europe and became a popular folk remedy. Its wide range of use as a general vulnerary and treatment earned it the name of “king’s cureall”. When the oil was first extracted from the evening primrose in 1919, Heiduschka and Lueft reported the presence of an unusual linolenic acid which yielded a hexabromide derivative of linolenic acid with very different physical characteristics from the other two common forms (?- and ?-isomers) found in linseed oil. Thus, they named it ?-isomer, or gamma-linolenic acid (GLA). The exact chemical structure of GLA was later characterized by Eibner et al. (1927) and verified by Riley in 1949. Since then, GLA has been found in many other plants, fungi, algae, and microorganisms. During the past 20 years, interest in the potential of GLA for medical use has grown rapidly, but not without great skepticism. Even the nutritional value of GLA was frequently criticized by some nutritionists. Nonetheless, the number of researchers engaged in GLA research has significantly increased, and, in many parts of the world, GLA has been accepted as a health food or a prescription drug. It was this continued growing interest in GLA that convinced the Health and Nutrition Division of the American Oil Chemists’Society that it would be most timely and informative to the public to convene an international conference to examine the biological value of GLA. Yung-Sheng Huang of Ross Products Division, Abbott Laboratories, and Mr. Frederick of Bioriginal Food and Science Corporation were given the responsibility of organizing this meeting. A particular strength of this book that, in many instances, the normal GI physiology is reviewed prior to the description of the abnormalities in DM. Purpose: The authors rightly assert that GI function has been inappropriately neglected, considering that it is intimately connected with glycemic control, and aim to rectify the situation by providing an impressive review of the data. Given the vast and ever increasing numbers of diabetics, this is a worthy objective. This book meets the authors' objectives of providing a comprehensive, concise review of the knowledge of GI function in the setting of DM. Audience: The book is largely targeted to clinicians and clinical researchers, according to the authors. It is targeted appropriately to this audience. It will be useful for both gastroenterologists and the endocrinologists. The contributors are among the world's most respected authorities. Features: The book begins with the epidemiology of gut symptoms in DM, reviews the animal data and then reviews each major division of the digestive system separately. A nice inclusion is a separate chapter on the relation between gut function and glycemic control. The organization of the book is excellent. Assessment: I see numerous patients with diabetes and GI symptoms and will find myself turning again and again to this reference for thoughts on how to manage these patients.
